We are so excited to share this experience with you! This questionnaire is so that we can best serve you the day of your portrait session. Just because this session is shorter than our typical 4 outfit session, we still want you to feel like the queen that you are and make the time here all about you! Please be honest, and there are no wrong answers! Fill it out and we will reach back out to answer any questions and set a date for the session. We are happy to hop on a call if that is easier for you. We are here to help every step of the way! You will be sent a complete Prep Guide to help you be totally prepared for the session.

What is your main reason for doing the session?

Would you describe yourself as modest, completely comfortable naked, or somewhere in the middle?

Would you say that you are comfortable in front of the camera?

What parts of your body do you love?

These sessions will allow for up to two outfits, do you plan to do the optional sheet set as well?

Do you understand the session fee is 175? Optional hair and makeup is an additional 200 and must be paid in cash to Michelle the day of the session.

Do you understand you must pay the session fee to secure your spot and the purchase of the minimum "Little Black Book" (800) must be paid by the day of the session? You will also receive 15% off any upgrades at your ordering appointment.
